UTHSC-H MD & DDS Graduates

All students expecting to graduate must submit an Application for Degree Form to the Office of the Registrar during registration for classes for the term you expect to complete requirements for the degree. The Graduation Fee published in the Tuition and Fee Schedule will be assessed at this time. The fee is non-refundable. Diplomas are pre-ordered, with the graduate's name based on the Application for Degree Form or the student's name in the Student Information System. Name changes after the diploma is ordered may require reprinting of the diploma and will be charged to the student. Once the requirements for degree have been met, the diploma is delivered to the school for delivery to the graduate during the graduation ceremonies. Any diploma which cannot be delivered during the graduation ceremonies may be picked up in the Office of the Registrar, or may be mailed to the graduate. A diploma will not be released to anyone other than the graduate without the graduate's written consent.
