Under the Texas Public Information Act, I'd like to request an electronic listing of all current employees of The University of Texas Health Science Center of Houston.

Specifically, I'm seeking a spreadsheet, database or delimited text file(s) — not paper records or a PDF created from data — with the following fields for each employee: Name (split last, rest if possible), title, department, race, gender, hire date and gross annual salary. (If you cannot produce all of these fields, please let us know.)

In addition, please include any lookup tables that may be needed to interpret the data. For example, if departments are listed with codes linked to a separate table, we'll be happy to join the records ourselves, rather than require your staff to do so.

I want to stress that I'm not seeking any records that are prohibited from release either by statute or previous rulings by the Texas Attorney General or the courts. Please let me know if you feel any records in the fields I've requested are confidential, and I'll consider amending my request.

Also, please let me know if clarifying the request might speed the release of the data, reduce any programming time and, most important, avoid a referral to the Attorney General.

We're comfortable working with large data sets and can receive the records in a variety of formats (.dbf, .csv, .mdb, etc) and media (CD, DVD, ftp).

I seek this information in the public interest as a journalist, and not for a commercial purpose. Therefore, I'm requesting that you waive any fees, as allowed under the Act. If this is not possible, please provide a detailed list that breaks down each charge that equals the total amount.

I understand UT System addresses TPIA requests to UT entities, so I am including the UT System Public Information Coordinator on this email.

Pursuant to the Freedom of Information Act, Under the terms of the D.C. Code, Section 2-532(c) and (d) and/or Tex. Att'y Gen. ORD-52 (1974); Tex. Att'y Gen. ORD-7 (1973) stating that canceled checks are public record, the following is a request for a list of any and all of the Health Science Center at Houston Outstanding/Voided Warrant and or Stale-dated checks, which may also be known as Limited Payability Cancellation Reports of refunds and credits (Undeliverable checks); specifically checks for businesses and the undeliverable refund checks for individuals, businesses and vendors that are older than 6 months old and claimable though your office. If possible, please indicate or eliminate any checks that have been or are presently being reissued.  Please limit the list to checks that are outstanding and 6 months old or older and amounts greater than $1500.00.  

I understand form other agencies that certain fields on the report may be confidential so we restrict our request to the check number, check date, amount of the check, agency code or reference number, and payee name and address.  However, if it is simpler and more cost effective to send the entire report, we can work around the extra fields.

SDC is requesting the complete personnel file for Mr. Jose Guadalupe Luna. Mr. Luna was an employee of The University of Texas System working at the parking garage owned by Prairie View A&M University (PVAMU) in Houston, Texas. The University of Texas Health Science Center at Houston, a component of The University of Texas System had a Parking Garage Management Agreement with PVAMU at the time of the claimed misappropriation of parking revenues.

This is a Public Information Request for check registers from 2009 – present 2013 for University of Texas Health Science Center at Houston, in pursuant to Texas Government Code Chapter 552.

Check registers for vendor payments should include: Vendor Name, Payment Date, Payment Amount and Payment Description (if available).  I prefer them in excel but pdf will do, or any other electronic format that is available. Please do not send paper copies.
