To Whom It May Concern:
This request is pursuant to Section 552.001 of the Texas Government Code, Title 5 Open
Government, Ethics, Subtitle A-Open Government, Public Information.

All requests for information and documentation pertain to number 1 and number 2 above.
Request is hereby made for any and all documents or information pertaining to:

1. The determination of the amount of monetary compensation for number 1 and
number 2 above in the years 2010, 2011, 2012, 2013 and through July of 2014.

2. The known sources of compensation.

3. In the years 2010, 2011, 2012, and 2013, the annual, semi-annual, monthly,
weekly, or daily payments for compensation.

4. The total amount of compensation from 2010 through 2013 which is the result of
their work as a practicing physician, separate from their teaching requirements at
UT, university or medical school.

5. The total amount of compensation from 2010 - 2013 which is the result of work
for the teaching requirements at UT - the university or medical school.

6. The criteria for determining compensation for the years 2010 through 2013. In
other words, does the compensation depend on productivity, hours worked, classes taught, lectures done, profitability of medical practice, money intake as a result of the doctor's regular or on-call medical practice, number of patients treated, or any other related or similar factor.

7. Any information or documentation showing the total compensation paid in the
years 2010 through 2013.

8. Any information or documentation showing the total compensation through the
end of July of 2014.

9. Any information or documentation concerning the credentials, curriculum vitae,
fellowships, academic achievements, professional writings, or board certification.

10. Any information or documentation showing you promote either number 1 and
number 2 above as having board certification with the American Burn
Association.

11. Any price list, criteria for pricing, income from or other information or
documentation concerning their work as paid medical expert witnesses involved
directly in or related to litigation matters.

12. Any information or documentation concerning the allowing of number 1 and
number 2 above to work outside the bounds of, outside the control of, or adjunct
to either the University of Texas Health Science Center or the Plastic Surgery
Out-Patient Clinic at 6410 Fannin, Suite 1400, Houston, Texas 77030.

13. Any known compensation for number 1 and number 2 above paid separately to
number 1 and number 2 above outside the bounds of the University of Texas
Health Science Center or its Plastic Surgery Out-Patient Clinic. In other words,
did either number 1 or number 2 above receive compensation directly from
litigants, hospitals, emergency facilities, outside lecturing, or anything similar?

14. Financing data to include all sources of information (including but not limited to
salary, benefits, extra pay, professional fee collections, contracts, with source,
grants, support supplements-with source, directorships, teaching (and expenses)
including, but not limited to, clinic expenses (PLI).

15. Any information or criteria regarding the compensation for number 1 and number
2 above for the years 2010 through July of 2014 that is based on duties in the
organization, leadership or any other similar criteria.

16. Any information or documentation concerning any and all compensation for
number 1 and number 2 above that would allow the organization to vary the
pay/compensation of either number 1 or number 2 above dependent upon
performance, leadership, workload, productivity, teaching, hours of work,
responsibilities, tenure, or other similar factors.

17. Any and all contracts of employment, memoranda of compensation, job duties,
job expectations and/or job descriptions.

18. Any and all income to the UT from the work of number 1 and/or number 2 above,
i.e., testimony provided, speech/lectures, public relation duties performed or
similar activities.
