Request for Waiver of Student Service Fees

To request a waiver of student service fees, please fill in and submit the form below. To be considered, this form and supporting documentation must be submitted by the 12th day of the fall/spring semester or the 4th day of the summer term.

Note: School of Public Health students attending satellite campuses are not exempt from paying student service fees and should not submit this form.

Note: Students in the SPH Certificate Program or the Dental Hygiene Bachelors Completion Program are not assessed student service fees and do not need to submit this form.

  1. NOTE: To secure an exemption due to concurrent enrollment, you must send a copy of your paid tuition receipt from the other UT institution by the deadline indicated above to Auxiliary Enterprises (Ronda.A.Mullane@uth.tmc.edu).
