Medical Records: If you need a copy of your medical records, please complete a release form and we will forward it to Medical Records.

Palliative Medicine Clinic at LBJ Community Hospital

This clinic utilizes an interdisciplinary team approach in providing symptom control and supportive care in the outpatient setting for patients with life-threatening illnesses. The team includes physicians, nurses, social workers, chaplain, and other health professionals. Clinic appointments are offered on Wednesdays from 8:00 am to 11:30 am and on Thursdays from 12:30 pm to 4:00 pm. The clinic is located on the first floor in the Outpatient Clinic area.

    The Bridge Program – House Call Program

    The LBJ Geriatric Clinic has a House Call Program designed for patients who are unable to come to an outpatient clinic. The House Call program staff provide medical care at the patient's place of residence and help to coordinate care with other providers, as needed. The goal of the house call programs is to prevent hospitalizations and to assist elders to maintain a healthy quality of life at home.

    Services include but are not limited to:

    • Complete geriatric assessment
    • Screening and treatment for common geriatric illnesses and syndromes such as dementia, diabetes, heart problems, osteoporosis, arthritis, functional decline/falls, and malnutrition.
    • Evaluation and management of medications
